Nothing on the network has changed that I'm aware of. Now when they try to fax to their user folder on the server it gives them an error, "Failed to send via SMB.

I am at my wits end. The users have a semi-work around in that they can use the fax-to-mail function in the interim. But the reason they setup the fax-to-file option was because some of the documents they scan are massive and will not send through email successfully since they're too large. Hate it when things stop working for no apparent reason. I assume this is a windows server and windows clients? It sounds like you have ruled out permission based errors, but you may want to check the security log on the windows server to see if it is stopping there.

If you use an AD service account for faxing, check to see if it is locked out or the password has expired. Microsoft pushed a lot of updates this past tuesday. If the server ws updated earlier last week, check for the updates that were applied. Yes, server is Windows and machines are Windows 7. I'll check those items today, thanks for the suggestions.
